Access to medical records

The practice is registered and complies with the Data Protection Act 2018 (DPA 2018). Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. If you wish to obtain a copy of your medical records, please complete the Subject Access Request (SAR) form below and return it to our surgery via email (riversidesurgery@nhs.net), post or in person. Once you have completed the form we will prepare your medical records for you to collect from our surgery. You will need to provide some form of ID when collecting your medical records.

Your medical records for solicitor or insurance claims

We are aware that patients will sometimes require access to their medical records for the purposes of legal claims. If this is the case, we should not and will not release your GP record to anyone but yourself – which is in accordance with guidance from the Information Commissioner’s Office.

If you require your medical records for solicitor or insurance claims we will still require you to complete a Subject Access Request (SAR) form (as above) and provide some form of ID. We will then prepare your GP record for collection.

Accessing your medical records online

If you have signed up to Patient Access, you will be able to see your medical records online, download and printout whatever you need without having to complete the forms above. To sign up to Patient Access now, please click here.

Proxy access

Proxy access is now set up for online patient access which allows parents, family members, carers, care home staff to have access to patient’s online service accounts – enabling you to request repeat medication, view medical records and where available book appointments on behalf of a relative or patient.  To enable this please complete this form – Patient Registration Form for Proxy Access to Online Services and hand it into our reception.
