Your health information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. GDPR – how we use your information
- We collect and hold data about you for the purpose of providing safe and effective healthcare.
- Your information may be shared with our partner organisations to audit services and help provide you with better care.
- Information sharing is subject to strict agreements on how it is used.
- We will only share your information outside of our partner organisations with your consent.
- If you are happy with how we use your information you do not need to do anything.
- If you do not want your information to be used for any purpose beyond providing your care please let us know so we can code your record appropriately.
- You can object to sharing information with other health care providers but if this limits your treatment options we will tell you.
- Our guiding principle is that we are holding your information in the strictest confidence.

Information for health – your records
The practice is registered and complies with the Data Protection Act 2018. We respect your right to privacy and keep all your health information confidential and secure. For further information please see our Riverside Surgery Privacy Notice 2023-24 and the Summary Care Record consent form, which you complete when registering with our practice.
Giving consent to a relative / next of kin
If you would like a relative / next of kin to speak to us on behalf of you, please complete the Information Sharing Agreement Between Patient And Carer Riverside Surgery form and return it to us so that your relative / next of kin, are authorised to speak to us about your medical information, test results etc. Unless this form is completed and returned to us, we will not be able to discuss your health with them. This is an important part of patient confidentiality, to protect all of our patients.
Proxy access
Proxy access is now set up for online patient access which allows parents, family members, carers, care home staff to have access to patient’s online service accounts – enabling you to request repeat medication, view medical records and where available book appointments on behalf of a relative or patient. To enable this please complete this form – Patient Registration Form for Proxy Access to Online Services and hand it into our reception.
